If you don’t already have insurance and the dog has already been diagnosed, I don’t think you can get insurance for that specific surgery… But I could be wrong…
I dont know of any that will cover a pre-existing condition. In other words, if your dog was already diagnosed with cataracts, you wont find an insurance that will cover any treatment for it.
As for covering surguries.. most “pet insurance” companies dont really fully cover surguries anyways.. there may be some kind of a “discount”.. but none cover things 100%. Honestly, you are better off to just put the money that would be used on insurance into a savings Acct for vet expenses.. if the insurance costs you $30 a month, then put $30 aside each month. Its there in the bank, drawing interest.. if you need it, its there.. if you never needed it.. great, you have a ton of money saved.

pet insurance will not cover previous problems with your dog. they will request a check up and will state all existing problems with the dog.
